Scheduling

  • Our general scheduling line (512) 450-1300 is open M-F from 7:30 am – 5:00 pm
  • You may also call an individual clinic’s extension to schedule during their clinic hours: Contact Us Here!
  • Have your insurance card ready if you are a new patient.

New Patients

  • INSURANCE REFERRAL – If your insurance requires a referral from your primary care physician, contact that office and request the referral. Some offices may take 24-48 hours to approve and send the referral to us.
  • NEW PATIENT FORMS – Please fill out the new patient forms here in advance and bring them with you.
  • WHAT TO BRING WITH YOU – Driver’s License or valid ID, insurance information, referral if required, and any reports/X-ray/MRI/CT scan that might be helpful to your therapist if not in the ASM system.
  • CLOTHING – Please wear comfortable, loose fitting clothing that will allow your therapist access to your injured body part.
  • ARRIVAL – Please arrive 20 minutes early for new patient paper work if not already completed.

Arrival Time

  • We strive to provide you with the best care possible and depend on schedule adherence to make that happen. In order to maximize your one-on-one care, we ask that you arrive on time.
  • Please see our cancellation policy. You may sign and bring with you to your first appointment.

Payment

  • We will be collecting co-payments, co-insurances, and cost towards your deductible at the time of your appointment. We accept cash in exact change, but no coins, checks and all major credit cards.
